PERMISSION TO PARTICIPATE
I, as the parent or legal guardian of the participant, confirm that this individual is a minor pursuant to the Age of Majority & Accountability Act, and provide permission for them to participate in the Canterbury Community Association program or activity.

RELEASE OF LIABILITY and WAIVER OF CLAIMS
As a participant, and/or as the Parent or Legal Guardian of a child who is a participant, and/or on behalf of any person whom I have registered to be a participant, in a program or activity as indicated on this form, I agree to indemnify and hold harmless the Canterbury Community Association, any of its agents, and the City of Ottawa from all claims, demands, actions and causes of action, loss, costs or damages that the Canterbury Community Association, its agents, or the City of Ottawa may suffer, incur or be liable for in relation to any injury that I, my child, or any person I have registered for a program or activity, may suffer or cause to others in connection with my or my child’s negligence or actions, or the negligence or actions of any person I have registered for a program or activity, while participating in the program or activity. Furthermore, I/We hereby release, waive and discharge the Canterbury Community Association, its agents, and the City of Ottawa from all liability to our heirs, executors, administrators for all loss of damage and any claim or demands for such loss or damage due to injury to person or property.

En tant que participant, et/ou que le parent ou le tuteur légal d’un enfant qui est un participant et/ou au nom de toute personne que j’ai inscrite pour être un participant, dans un programme ou une activité, comme indiqué sur
ce formulaire, je conviens d’indemniser et d’exonérer l’Association communautaire de Canterbury, ses agents, et la Ville d’Ottawa et de les dégager de toute responsabilité relativement aux réclamations, aux revendications, aux causes d’action, aux pertes, aux frais et aux dommages dont celles-ci pourraient faire l’objet ou qu’elles pourraient encourir en raison de blessures que mon enfant ou moi- méme, ou toute autre personne que j’ai inscrite à un programme ou une activité, pourraient subir, s’infliger ou infliger à autrui, par négligence ou en participant auxdits programmes et activités. En outre, je libère et je dégage l’Association communautaire de Canterbury, ses agents, et la Ville d’Ottawa de toute responsabilité envers nos héritiers, exécuteurs, testamentaires, administrateurs à l’égard de toute perte ou de tout dommage et de toute réclamation ou revendication afférente auxdits pertes et dommages qui pourraient découler d’une blessure corporelle ou d’un dommage matériel.
